Deciding that long-term care is necessary for you or a loved one and selecting a long-term care home is an important and personal decision that requires careful consideration. To guide you through this process, we have compiled the steps involved in finding and selecting an appropriate long-term care home.

Step 1
Your first step is to carefully consider you or your loved one’s particular needs. Long-term care homes care for people who don’t need to be in hospital but can’t be cared for at home or in a retirement or assisted living facility due to illness, injury, frailty or cognitive limitations. Clearly identifying these needs will assist you in determining what services you or your loved one will require while in long-term care.

Step 2
Find the long-term care homes located in your area. Research the different services they offer. Click here to find a Leisureworld location in your area.

Step 3
Contact your local Community Care Access Centre (CCAC). Click here to find the Community Care Access Centre closest to you, or call 310-CCAC (2222). Once you locate your local CCAC, they will put you in contact with a Placement Coordinator or another staff member who will help you through the steps.

Step 4
Take a tour of the homes you are interested in to see how suitable each home is. During the tour, discuss you or your loved one’s particular needs with the home’s representative, and ask residents and staff what they think of the home.

Step 5
Select your top 3 choices of homes, which you will provide to the CCAC. The CCAC will work with you to plan the placement that is best for you or the person you care for. If your first choice is not available, the CCAC will help you find an appropriate alternative.

Step 6
When suitable accommodation becomes available, you will be contacted by the CCAC.

Step 7
If you accept the accommodation offer, the CCAC will coordinate the details of admission with the long-term care home.

The placement process is fair, providing equality of treatment to all clients. It is a process that is culturally sensitive and respectful of the dignity and choices of each individual. In the event you require services while awaiting placement, the CCAC will make the necessary referrals and connect you to the appropriate services and service providers.

There are three types of placement:

  • Long-term or permanent stay, the most typical.
  • Short-term, designed to support clients during recovery from an illness or to provide a respite break for a family caregiver. These stays are for a short, specified period of time up to a maximum number of days per year. A daily fee, set by the Ontario Ministry of Health and Long-Term Care, is charged.
  • Crisis placement is when immediate admission to a long-term care home is deemed necessary. In this case you will be placed in the first suitable home that becomes available. You may transfer to another home at a later date.
